Parental agreement and consent form for students aged under 18

Introduction

We want to make sure that your child is safe and happy while studying in the UK. To help us, we ask you (the parent or legal guardian) to complete this form for any student aged under 18 who is enrolled at English in Chester. For further information about how we use your child’s data, please look at our Privacy Policy.

Please discuss this agreement with your son/daughter and complete. The agreement should be signed and returned to English in Chester 4 weeks before the course.

If accommodation is NOT organised by English in Chester please supply the name, address and contact number of the person your son/daughter will be living with. If the person accommodating your child is not an immediate family member, by signing this parental consent form you confirm that you have carried out appropriate police checks and references.

Pastoral care

The level of care offered to your child is outlined on our website on the sheet called Information for parents or guardians of students under 18 years old. Travel

I understand and agree that:

  • my son/daughter can travel unaccompanied to and from Chester at the start and end of their course. (Please note that English in Chester advises all students under 18 to take transport arranged by the school. The school does not accept responsibility for students under 18 who make their own travel arrangements from the airport or other arrival point until they arrive in Chester.)
  • my son/daughter can travel unaccompanied between the school and his/her accommodation.
  • my son/daughter will seek permission for any travel outside of Chester during their stay.
  • I will need to give additional and specific consent for any overnight stays.
Attendance

I understand and agree that:

  • my son/daughter is expected to attend all scheduled classes and activities. If I wish my son/daughter to be absent at any time, I must contact the school.
  • he/she understands that he/she must follow the school’s ‘attendance policy’.
Curfew time

I understand and agree that:

  • my son/daughter must be home by their particular curfew time (please see below) on any evening unless accompanied by a responsible adult approved by the school, and to tell our them of this rule.
  • CURFEW TIME for students under 18 years old on General English course: 23:00
  • CURFEW TIME for students on the Summer Course for Teens: 20:00
Leisure activities

I understand and agree that:

  • my son/daughter can go on any trips/activities organised by the English in Chester or English in Chester’s homestay hosts and to take part in these activities, under supervision of staff/hosts.
Unsupervised time

I understand and agree that:

  • my son/daughter can have free time for shopping on trips.
  • my son/daughter can have unsupervised free time in Chester during the time between the end of classes/activities and the time of the evening meal at their accommodation.
  • my son/daughter can have unsupervised free time in the evenings and weekends.
Students aged 16 and 17 who are enrolled on adult courses

I understand and agree that:

  • my son/daughter will come into regular contact with other students over the age of 18, in class and during the leisure programme.
  • there are certain British laws (e.g. related to drinking alcohol) that apply to people aged under 18. As a consequence, there may be some leisure activities which my son/daughter cannot take part in because of their age.
Other

I understand and agree that:

  • it is illegal to buy alcohol under the age of 18 and illegal to buy cigarettes under the age of 18 or to smoke in public under the age of 16, and that the school will enforce these laws. If necessary, the school may search students’ belongings and confiscate illegal or other items deemed to be inappropriate.
  • we (the parents/guardians) are financially responsible for any damage that our son/daughter causes during his/her stay in the UK.
  • if the school believes that the safety of my son/daughter is compromised, English in Chester/homestay host may impose further restrictions or rules as required.
  • it is important to discuss staying safe online with our son/daughter and we have explained that he/she should no share personal information with strangers on the internet.
  • my son/daughter must have adequate travel and medical insurance and will ensure they have this before they travel.
  • my son/daughter understands that he/she must follow the school and homestay rules.
  • my son/daughter must follow to the school’s policy on unreasonable behaviour:
    • Please see the school’s discipline and exclusion policy.
    • In the first instance, our staff will speak to the student explaining why the behaviour is unreasonable. If necessary, the student will then receive an official warning in writing with a copy sent to the parents.
    • If unreasonable behaviour continues the student may be sent home immediately without a refund. The parents will be asked to pay for the return flight.
Medical issues and disabilities

Please tell us about any issues. If we are not told in advance about a physical or mental condition, to terminate the student’s course without refund.

I understand and agree that:

  • in case of minor pain or illness such as headache, mild cold or sore throat, my son/daughter may be given non-prescription medication such as Paracetamol, cough medicine, throat pastilles, antihistamine or travel sickness tablets.
  • in case of an emergency I give permission for a responsible person in the English in Chester or my child’s homestay host to arrange medical treatment. Of course, every effort will be made to contact you, the child’s parents/guardians as quickly as possible.
  • English in Chester may register my son/daughter with a doctor (General Practitioner) in the UK.
Photographs and video clips

As part of your child’s course, we may take pictures and video clips that include your child. These may be used internally in class work, on noticeboards/screens or on secure school blogs. These will not be shared externally without your permission.

Separately, we ask for your consent to use images of your child in School and School partners’ external marketing materials (social media, brochure, website and video). You can change your consent at any time by contacting the School, and we will take reasonable measures to comply as soon as possible.
